REQUIRED DOCUMENTS FOR ALL CLIENTS WITH GRACE NETWORK:
1. Picture ID (valid driver's license, passport, state photo ID or birth certificate)
2. Social Security Card for all household members (or number on an official paper)
3. Proof of income for EVERYONE in your household for the last and current months: paycheck stubs; letters from Social Security, SSI, child support; checking/savings account information; proof of any other income (Food Stamps, TANF, etc.)
4. Monthly expenses/bills needed to pay now (Grace Network will not pay for all of these, but needs to see them all to establish the crisis): rent/mortgage; medical expenses; gas, insurance bills (life, burial, car); furniture bills; electricity; fuel oil; cable bill; telephone; water; child support; loans, child care; car payment; credit card statements
5. Receipts for what has been paid during the past and current months
6. Proof of the crisis: doctor's statement; police report; late notice; cut-off notice; eviction notice; etc.
7. Rent/Mortgage Information: rent receipt; landlord notice; lease agreement; landlord's name and telephone number; Referral from Section 8 or housing authority; mortgage payment statement
8. Bank Account Information: checking and savings accounts, etc.

Must be a resident of Martinsville/Henry County.
No appointments. First come, first served basis. Some clients arrive up to an hour before the office opens.
